Through the Halton Hearing Centre's affiliation with Oakville's Dr. Jack Kolenda (ENT), you will not be charged for your Hearing Test as long as you have a referral for the appointment from your family doctor.

Download this form and print it from home. Simply have your physician complete the referral form and either:
a) Fax the form to us at 905-849-8770, or
b) Bring the form to us in person
Once the referral is received by our office, an appointment will be scheduled and your physician will be notified.
Our office hours are: Monday - Friday 9 am - 5 pm
